Targeted Plans of Care

Targeted Plans of Care are scheduled series of appointments that are designed to specifically address the symptoms bringing you to treatment. Your series of appointments will focus on a specific intervention or approach that has been scientifically proven to effectively treat your diagnosis. Targeted Plans of Care are collaboratively selected and individualized by our clients and their providers.

Insomnia

Cognitive-Behavioral Therapy for Insomnia (CBT-I) addresses sleep efficiency, behavioral impediments to sleep, and cognitive strategies for improving sleep. CBT-I sessions are offered individually or as a class.

We also offer Targeted Plans of Care for hypersomnia and for nightmares.


Depression

We offer 3 options for evidence-based Targeted Plans of Care for depression:

Cognitive-Behavioral Therapy for Depression (CBT-D) addresses unhelpful, unhealthy patterns of thinking, feeling, and behaving. CBT-D sessions are also offered as a class.

Acceptance and Commitment Therapy (ACT) emphasizes mindfulness and cognitive flexibility to embrace difficult circumstances.

Interpersonal Therapy (IPT) focuses on addressing symptoms by improving interpersonal functioning and strengthening relationships.

Anxiety

Cognitive-Behavioral Therapy for Anxiety (CBT-A) focuses on psychoeducation, breathing retraining, mindfulness, behavioral intervention, and cognitive restructuring to decrease symptoms of anxiety and panic.

Obsessive-Compulsive Disorder

Exposure and Response Prevention (ERP) focuses on both imaginal and real-life exposure to cues that trigger obsessive thoughts, followed by strategic prevention and mitigation of compulsions to relieve the anxiety associated with obsessions. 

Post-Traumatic Stress Disorder

We offer 2 options for evidence-based Targeted Plans of Care for PTSD. Both incorporate exposure, behavioral intervention, and cognitive restructuring.

Cognitive Processing Therapy (CPT) offers exposure in written form and is focused on cognitive-behavioral intervention.

Prolonged Exposure (PE) offers auditory exposure and is focused on accommodation and assimilation.

Adult ADHD Executive Functioning Coaching

Adults with ADHD typically struggle with inattention, distractibility, disorganization, planning, working memory deficits, emotion regulation, and difficulty fitting in with peers and colleagues. Executive Functioning coaching offers strategies to help adults navigate these challenges while focusing on individual strengths and building self-esteem. 

Chronic Pain

Cognitive-Behavioral Therapy for Chronic Pain addresses clients' thoughts, feelings, and behaviors relative to their chronic pain to help improve their quality of life by improving their management and experience of pain. 

 

Other Conditions

Targeted Plans of Care are also offered for patients experiencing panic disorder, phobia, tic disorder, and personality disorders.


Community Sleep Education Classes

We offer community classes for expectant and new parents to implement healthy sleep schedules. These classes consist of three 1-hour class meetings. A comprehensive intake session is not required to enroll in these classes. The ideal time to enroll in this class is in the third trimester of pregnancy or 8-10 weeks prior to the placement of a foster or adoptive child; however, if you already have a baby and are struggling with sleep, this class will be of benefit to you, as well.

We also offer community classes for clients struggling with CPAP use. In these classes, we provide instruction on CPAP adherence and desensitization.  These classes consist of three 1-hour class meetings. 

A comprehensive intake session is not required to enroll in these classes.
